This page contains links to some of the FAA Forms required in conjunction
with Federal Aid Grants through the Airport Improvement Program
(AIP). Information and forms provided on this page are provided
for the benefit of airport sponsors and consultants.
These forms are in a Microsoft Word format. This format, as opposed
to a read only format, permits users to input information onto the
forms using available word processing software. When available,
files will also be provided in the read-only Adobe Acrobat format.
While users may be permitted to manipulate the forms in order to
suit their specific project requirements, users are cautioned that
the FAA reserves the right to reject any modification made to any
form that will alter the basic intent of the form. Please consult
regional personnel if you have a question regarding this matter.
As additional forms become available in electronic format, the
links will be posted on this page. We welcome comments from our
Sponsors and Consultants. Ideas, suggestions, or request for specific
forms may be forwarded or E-mailed to the FAA Airports

Additional information on FAA Form 7480-1
Must be filed with the FAA at least 90 days before any of the following
activities (exception: if associated with an active AIP project,
filing is not required for items 2 and 3)
- Construct or otherwise establish a new airport or activate an
airport.* **
- Construct, realign, alter, or activate any runway, or other
aircraft landing or takeoff area of an airport.*
- Construct, realign, alter, or activate a taxiway associated
with a landing or takeoff area of a public-use airport.*
- Deactivate, discontinue using, or abandon an airport or any
landing or takeoff area of an airport for a period of one year
or more.
- Deactivate, abandon, or discontinue using a taxiway associated
with a landing or takeoff area on a public-use airport.*
- Change the status of an airport from private use (use by the
owner and other persons authorized by the owner) to an airport
open to the public or from public-use to another status.
- Change status from IFR to VFR or VFR to IFR.
- Establish or change any traffic pattern or traffic pattern altitude
or direction.
*Also requires a sketch showing changes. **Also requires showing
the airport on a 7.5 minute quadrangle map.
