Disposition of Applications and Medical Examinations

All completed applications and medical examinations, unless otherwise directed by the FAA, must be transmitted electronically via AMCS within 14 days after completion to the AMCD. This includes International AMEs.

In addition, the FAA/Original Copy must be mailed to:

Civl Aerospace Medical Institute
Federal Aviation Administration
P.O. Box 26080
Oklahoma City, OK 73125-0080
These may be batch mailed at monthly intervals. All incomplete applications and medical examinations must be mailed immediately to the above address.

The AME Work Copy must be retained by the Examiner as their file copy for at least 3 years unless exceeded by state law requirements where the Examiner is licensed and performed the examination.

The Applicant's Copy of the FAA Form 8500-8 (last page) must be given to the applicant along with the information for Applicant and the instruction sheet.
