Listed below are links to assist you in reporting your FAA drug and alcohol testing results for 2009. You have an option for electronic reporting and an option for filling out paper forms for submission. If you use the paper-reporting format, please send the information to the address provided below. The FAA highly recommends the electronic reporting option.
The DOT's Volpe National Transportation Systems Center (Volpe Center) will be assisting the FAA in collecting data from the MIS reports. In December, the Volpe Center will be sending letters to all aviation companies that we believe are required to submit reports for 2009. These letters provide user ID, password, and instructions for electronic reporting. OLDER VERSIONS OF THE MIS FORM (FROM BEFORE 2003) CANNOT BE ACCEPTED FOR 2009 REPORTING.
