Replace a Knowledge Test Report

There are two ways to replace your knowledge test report. You can mail us:

• an Application for Replacement of Lost or Destroyed Knowledge Test Report(s) (PDF) form
• or a signed, written request stating your:
  • name
  • date of birth
  • social security number
  • type of test
  • date you took the test

You can mail the form to:

You must include a check or money order for $1 (U.S. funds) made payable to FAA. We can't:

• accept facsimile requests for test results
• fax test scores
• give test scores over the phone
