Description

This form is to be used by individuals seeking to operate certain small aircraft in accordance with Chapter 14 of Code of Federal Regulations (14 CFR), Section 61.113(i), BasicMed. This rule allows airman to meet medical eligibility requirements for certain operations without holding a third-class FAA Airman Medical Certificate.

Note: This form is in the process of being updated to reflect the current operations which may be conducted under BasicMed. While the existing form reflects the original limitations of BasicMed prior to expansion of BasicMed by subsequent rulemaking, airman may use this version of the form while exercising the expanded privileges.
