Airmen Certification
There are two ways to get copies of your airmen certification records. You can mail us:
- Form AC 8060-68 – REQUEST FOR COPIES OF MY COMPLETE AIRMAN FILE form
- or a signed, written request stating your:
- name
- date of birth
- social security number or certificate number
You can also request release of your records to third party Form AC 8060-69 – REQUEST FOR COPIES OF MY COMPLETE AIRMAN FILE TO BE RELEASED TO A THIRD PARTY.

Once you receive your file you will be notified of the total charges due and payment options.
In order for an airman or third party to obtain copies of medical records or a duplicate medical certificate, the request should be mailed to:

Please note: The Pilot Records Database (PRD) is now available for ATP, Commercial or Remote pilot certificate holders to view their pilot records or grant consent to an air carrier or proxy to view their pilot records for purposes of employment. This portal allows the pilot or air carrier access to review pilot certificates, ratings, medical certificate and other information provided by the airman. For details please visit the PRD site.
