There are two ways to get copies of your airmen certification records. You can mail us:
- Request for Copies of My Complete or Partial Airman File form
- or a signed, written request stating your:
- name
- date of birth
- social security number or certificate number
You can also release copies of your airman certification records to a third party.
You can mail all your requests to:
