- Oxygen required when flying above 7,999 feet.
- Passenger(2) carrying prohibited.
- Not valid for pilot in command.
- Not valid for night flying or by color signal control.
- Not valid for night flying.
- Must wear corrective lenses, possess glasses for near/intermediate vision.
- Holder shall possess glasses for near/intermediate vision.
- Not valid for flying where radio use is required.
- Valid for flight service station or center duties only.

- Second class privileges limited to aerial application.

- Valid for 6 months following the month examined.
- Valid for 9 months following the month examined.
- Valid for 12 months following the month examined.
- Valid for 15 months following the month examined.
- Valid for 18 months following the month examined.
- Valid for 21 months following the month examined.
- Valid for 24 months following the month examined.
- Not valid for carrying passengers or cargo for compensation,
- except if serving as pilot of fully qualified 2-pilot crew.
- Not valid for any class after (enter the expiration date in the field to the right)
- Not valid outside the borders of the United States.
