This form is for law enforcement to electronically submit referrals to the FAA regarding potential drone violations. The FAA will use this form to expedite enforcement proceedings against individuals who operate a drone contrary to FAA regulations. The FAA will contact the reporting law enforcement officer should additional information be needed in order for the FAA to proceed with legal enforcement actions.
At the end of the encounter, tell the individual they will be hearing from the FAA and that they can register their drone at https://www.faa.gov/uas/getting_started/register_drone.
This form should only be used by law enforcement and/or public safety officials. By submitting this form, you are certifying that the information provided in, and in any attachments to, this UAS Incident Report Questionnaire is true, accurate, and correct to the best of your knowledge and belief. The information submitted will be relied upon by the FAA Administrator in investigating the matter described in the UAS Incident Report Questionnaire. An individual, in any matter within the jurisdiction of any department or agency of the United States, who knowingly and willfully falsifies, conceals, or covers up by any trick, scheme, or device any material fact(s), statement(s), representation(s) or entry(ies) may be fined up to $500,000 or imprisoned for not more than five (5) years or both (18 U.S.C. §§ 1001 and 3571).
